This technique can be used in all types of perforations. Instead of only 180' flap and annulus elevation, in the technique the flap is elevated 270' and often annulus elevated all around 360'.

Advantages:

1. Technically easy and less time consuming.
2. No need to created an anterior window to anchor the graft.
3. Avoids anterior blunting of the graft.
4. Provides good exposure for atticotomy or attico antrostomy, if required.
5. Graft in contact with bony canal atleast 3/4th of the canal, therefore better graft stability and healing.
6. Graft is in contact with Tympanomeatal flap as well as Modified Koerner's healing, therefore better blood supply, healing and graft stability.
7. Anchors the graft well all around in subtotal as well as total perforations.
8. Easy access of bony canal for canalplasty, if required.
8. Over all results are very good.
